Quality of Life and Survivorship - Implementation Team

The Quality of Life and Survivorship Implementation team is focusing on survivorship planning for the 2009/2010 year. We are working to develop and brand a Survivorship Care Plan that can be used at each cancer center statewide to encourage consistent care for cancer survivors across the state.
Survivorship Care Plans outline the treatments the cancer survivor has received, any reactions to treatment, the outcome of treatment, expected/possible late effects and suggested follow up care based on the NCCN guidelines and on the oncologists observations. Survivorship Care Plans are shared with each cancer survivor, and the primary care physician to improve healthcare throughout the continuum of life. We hope to present the survivorship care plan to each cancer center in the state and gain a commitment from at least two cancer centers to include the survivorship care plans in their continuum of care.

Treatment & Research Group Implementation Team

- Work to ensure prevailing standards of care, accessibility, availability, and utilization of cancer treatment services for all Montanans and to promote optimum patient-provider communication

- Recommend the priority strategies and a timeline for implementation for the Treatment and Research sections of the CCC Plan
- Identify tools, projects, best practices and resources to help with implementing the priority strategies of the Treatment and Research sections

Early Detection Implementation Team

The Early Detection and Implementation Team’s (EDIT) priorities for 2009- 2010 focus on promoting and incorporating early detection cancer screening guidelines into workplace wellness programs and other community–based health–related education programs. Colorectal cancer is a specific focus of the Early Detection Team as this cancer is the fourth most common newly diagnosed cancer in Montana. Colorectal cancer is curable if it is diagnosed early through colonoscopy screening. Adults age 50 and older should be screened regularly for colorectal cancerBy collaborating with various stakeholders across the state and the MTCCC, the Early Detection Team is working to emphasize the importance of early detection and prevention of cancer through screening and healthful living. EDIT’s goal and strategies for 2009-2010 are twofold 1) address known underutilization of cancer screening services and promote culturally appropriate competent education 2) Broaden coverage and utilization for cancer-screening services in Montana.

Prevention Implementation Team

The MTCCC Prevention Team 2009-2010 priorities focus on the following; support for tobacco control and full implementation of the Clean Indoor Air Act, working towards integration with other chronic disease programs, support for existing nutrition and physical activity programs, and researching evidence-based skin cancer prevention strategies for the state of Montana.We work closely with the Montana Tobacco Use Prevention Program and MSU’s Nutrition and Physical Activity Program to encourage Montanans to make lifestyle choices proven to decrease an individual’s risk for cancer. Prevention is a key element in cancer control, and the MTCCC Prevention Implementation Team is working hard to promote cancer risk modification for all Montanans.
Key Objectives and Strategies:
- Work to reduce the risk factors that lead to cancer: tobacco, poor nutrition, inactivity, obesity, ultraviolet light exposure, and environmental carcinogens
- Recommend the priority strategies and a timeline for implementation for the Prevention section of the CCC Plan
- Identify tools, projects, best practices and resources to help with implementing the priority strategies of the Prevention section
Resources and Membership Committee

The Resources & Membership Committee strives to facilitate the implementation of the MTCCC Plan by seeking out needed resources including human, in-kind, and monetary. The group supports MTCCC membership development by recruiting new members and keeping current members engaged. When new members are recruited, the group matches them to MTCCC implementation teams and standing committees by assessing their skills and interests to best utilize their particular talent. The group also works to identify inkind and monetary resources to support the implementation of the MTCCC Plan through the teams and committees yearly activities. The work we do supports the many conference calls, outreach activities, educational campaigns, and meetings held throughout the year. The Resources & Membership Committee is the bedrock of the Montana Cancer Control Coalition, and we need your help to continue the life-saving mission of the coalition.

Assessment and Development Standing Committee

The MTCCC Assessment and Development Committee keeps each of the MTCCC Implementation Teams and Standing Committees on track in their efforts to implement the Montana CCC Plan. The Assessment and Development Committee gathers supporting data, evaluates, and analyzes each group’s activities and outcomes to ensure the activities will be successful and in line with the Montana CCC Plan, which in turn is helping to forward the vision, mission, and goals of the MTCCC. The Assessment and Development Committee measures the progress the MTCCC has from year to year. It is the Assessment and Development Committee who keeps the MTCCC on the same page and ensures we are helping to reduce the burden of cancer here in Montana.

Communication Standing Committee

The Communications Committee develops effective communication strategies to promote the MTCCC mission to lessen the burden of cancer in Montana. We help develop the appropriate tools and technical assistance to highlight the activities of the coalition teams and workgroups within the Coalition, as well as promote these Coalition activities to the general public. The Communications Committee has been successful in the development and launch of the MTCCC website, which serves as a rich resource for both members and the public at large. We support internal communication efforts with the dissemination of the MTCCC newsletter, keeping coalition members aware of new cancer control information and activities. In May 2009, we provided educational outreach to promote the importance of colorectal cancer screenings by hosting the Prevent Cancer Super Colon event, at which we had over 350 visitors tour the 20 ft. colon replica to learn more about prevention and early detection. To further our outreach efforts, we are currently creating a media toolkit which will serve as a comprehensive resource for coalition member-use for the promotion of MTCCC statewide activities. This kit will include items such as press release templates, posters, power point presentations, business cards, etc… All efforts taken by the Communications Committee are aimed to increase the exposure of MTCCC in our communities, as well as to maintain efficient communication networks among the various coalition teams, workgroups, and members at large.
Key Objectives and Strategies:
- Develop Internal and External communication short and long range plans, media response, and make recommendations to the Steering committee
- Tasks include work on the CCC Plan Advocacy section Goal I Objectives 1 & 2 with the Education and Legislative Support Expertise Workgroups
